This project documents AI chatbot incidents to help researchers, journalists, policymakers, and the public understand the current limitations and failure modes of AI systems.

What We Are Looking For

Service reliability issues: Outages, errors, or unexpected behavior
Safety concerns: Inappropriate, harmful, or concerning AI responses
Enterprise or business impacts: AI failures affecting organizations
Any documented incident with verifiable details
